Job Description: We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our home health care team. The LVN will provide nursing care to patients in their homes, ensuring they receive the best possible care. This role requires a strong commitment to patient care, excellent communication skills, and the ability to work independently.

Key Responsibilities:

• Provide direct patient care under the supervision of a Registered Nurse (RN) or physician.
• Perform assessments and create care plans for patients.
• Administer medications, treatments, and procedures as prescribed.
• Monitor patient health and report changes to the supervising RN or physician.
• Educate patients and their families on health care needs and instructions.
• Maintain accurate and timely documentation of patient care and services.
• Coordinate with other health care team members to ensure comprehensive care.
• Follow infection control procedures and safety protocols.

Qualifications:

• Current LVN license in the state of Texas.
• Minimum of 1 year of nursing experience, preferably in home health care.
• CPR certification.
• Strong clinical skills and knowledge of home health care practices.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Reliable transportation and valid driver’s license.
