Key Responsibilities

• Deliver compassionate, high-quality care to hospice patients in their homes, nursing facilities, or hospice care centers.
• Perform thorough patient assessments and develop individualized care plans in collaboration with the interdisciplinary team.
• Manage and administer pain and symptom management treatments to promote patient comfort and quality of life.
• Educate patients and families on the end-of-life process, symptom management, and care strategies.
• Provide emotional support and guidance to patients and families, fostering a calming environment.
• Maintain accurate and timely patient records, ensuring compliance with all regulatory requirements.

Qualifications

• License: Active Registered Nurse (RN) license in the state of Texas.
• Education: Nursing degree from an accredited institution (BSN preferred).
• Experience: Minimum of 1 year of experience in hospice, palliative care, or a related field.
• Skills: Strong communication, emotional intelligence, and patient-centered care approach. Ability to work independently and as part of a team.

Work Environment

• Deliver care in patients’ homes with a flexible schedule.
• Travel locally to serve patients in your community.
• Physical activity includes prolonged standing, walking, and lifting up to 50 lbs.
